Volunteers of America South Central Louisiana is hiring a Homeless Outreach Case Manager for the Baton Rouge area.
 

The Outreach Case Manager must be able to conduct street outreach as scheduled by the Program Director. They must be able to conduct street outreach to fulfill a contract with EBR Libraries and the LABOSCOC- Outreach contract. The Outreach Case Manager will be responsible for linking participants to referrals and community services in EBR. The Outreach Case Manager must be able to complete intakes an initial assessments.  They must be able to input data into the HMIS (Homeless Management Information System), and input coordinated entry data into the HMIS system. The Outreach Case Manager must be able to drive the mobile outreach van. They must be able to assist program individuals in obtaining community support, resources, and services. They must be able to complete monthly and quarterly program reports, maintain training requirements, and projects and duties as assigned.  

JOB REQUIREMENTS:
A minimum of a High School Diploma and experience with homeless population and/or individuals with disabilities required. Local travel, a valid driver's license & current vehicle insurance as well as a clear driving record are a must.  Experience working in HMIS database preferred but not required.
