Job Responsibilities:
  • Provides one-on-on services for adults and/or children with behavioral health disorders
  • Provides training and support that promote recovery
  • Services primarily provided in community
  • Conducts assessments of persons served
  • Completes intakes and develops individual service plans
  • Monitors the following community living areas: housing, entitlements, education, finances, social/recreational, family living, mental and physical health, legal assistance 
  • Provides and documents job mandated services to meet current program budgetary goals 
  • Links persons served to needed services and acts as advocate as needed 
  • Creates, develops, and maintains linkages with other service providers and community resources that may be of assistance to persons served 
  • Manages all case records in accordance with licensing and contractual requirements using an Electronic Health Record 
  • Develops and maintains positive relationships with funding and regulatory agencies
  • Flexible schedule
Position Requirements:
  • Bachelors degree in psychology, social work, counseling or sociology required
  • Experience in the provision of direct client services in a social service related field required
  • Must have safe, reliable transportation and ability to transport persons serviced in personal vehicle within program service area
