Description

Summary: Responsible for maintaining a small caseload and supervising Ryan White and MAI Case Managers and Consumer Advocates

Position Type: Full Time, Exempt

Essential Functions

  • Maintain a small caseload.
  • Assist with the supervision and management of daily operational activities of the case management team.
  • Assist with case supervision to ensure home visits, charting, data entry and follow-up are conducted.
  • Assist with quality management and review of Ryan White Case Management Services
  • Assist with grant writing to ensure continuity of funding and grant performance reports.
  • Review and approve time sheets and mileage logs.
  • Serves as backup to Program Director and staff.
  • Attend all Ryan White Mandatory Meetings and Trainings
  • Follow all guidelines & procedures related to EZ Labor (clocking in & out, reviewing timecard, requesting edit sheets, etc.)
  • Maintain professional, courteous relationships with all consumers, staff, community, members and professional partnerships
  • Maintain training requirements.
  • Report any significant concerns to Program Director
  • Provide 24-hour on-call supports.
  • Projects and duties as assigned.

Supervisory Responsibility

RW/MAI Case Managers and Consumer Advocates

Required Education and Experience
Must have a Bachelor’s degree in Social Work or Human Services field and at least two years of paid post degree experience in providing case management services.  Job involves local travel so a valid driver's license and vehicle insurance is required.
