Volunteers of America is seeking to hire a Program Coordinator/Administrative Case Management in the Lafayette area.
Responsible for assisting seriously mentally ill clients with the SSI application process, medical records collection, and collaboration with the Social Security Administration, to ensure a high percentage eligibility rate.
Essential Functions:
- Screen referred clients for potential SSI eligibility to include an income and resource assessment and determine eligibility for government benefits.
- Provide transportation to clients for the application phase of the SSI eligibility process.
- Advocate on behalf of the client with the interviewing process with SSA representative.
- Assist clients in completing additional forms and questionnaires as needed for their SSI claim.
- Collect all documented medical records concerning each client’s claim and ensure they are supplied to Disability Determinations. Compile all records into a single report depicting the client’s psychiatric and medical history.
- Review at least monthly each client’s status with SSA claims.
- Attend training as per regulatory requirements.
- Submit an active client progress report monthly to OBH which includes current application status and latest action taken.
- Maintains and submits proof of active auto liability insurance for privately owned vehicles used to conduct agency business and/or transport agency clients. Employees must submit proof of insurance prior to employment and each time policy is renewed thereafter. Failure to maintain active coverage and submit proof may result in disciplinary action.
- Transports clients as directed and follows Agency standards, rules, regulations and all laws for safe transport.
- Prepare monthly invoice with supporting documentation and submit to OBH.
Required Education and Experience:
This position requires that an individual have a high school diploma or the equivalent. The individual must possess a Louisiana driver’s license and have access to an insured automobile. Knowledge of the Social Security process preferred.
