Volunteers of America South Central Louisiana is seeking to hire a Therapeutic Foster Care Case Manager/Recruitment Specialist in Baton Rouge


The TFC Case Manager/Recruitment Specialist is responsible for providing support services to TFC families and children in foster care, as well as targeted recruitment and training of foster parents who are ab le to effectively and supportively meet the individual specialized needs of TFC youth, including youth who have been victims of human trafficking, youth who demonstrate sexualized behaviors, trauma and loss issues, developmental and cognitive delays, and antisocial behaviors and conduct disorder.  The TFC Case Manager reports to the Therapeutic Foster Care Program Director.

Essential Job Functions:
• Develop a comprehensive treatment plan within 15 days of a child placement and in collaboration with the child and family, DCFS staff, and TFC Program Director.
• Conduct a minimum of twice per month home visits to each youth and TFC parent on the Case Manager/Recruitment Specialist’s caseload, and provide 24-hour crisis intervention services, as needed, to assist foster families. This can be done via telecommunication or in person, per the situation.
• Maintain youth and parent records in accordance with all federal and state regulations, licensing requirements, and Therapeutic Foster Care policies and procedures, which include contractual requirements of DCFS Child Placing Agency Standards.
• Provide case management services for TFC youth and families and make referrals for any additional educational, medical, dental, social, independent living skills, or clinical services, as indicated, and upon discharge from the program.   Communicate these referrals to the DCFS Case Worker and TFC Program Director in a timely fashion, both verbally and in writing.
• Attend and participate in DCFS Family Team Meetings.
• Attend and participate in Individualized Education Plan (IEP) meetings along with the child and therapeutic foster parents.
• Monitor and assure that the TFC child receives routine medical care, including medication monitoring and administration as indicated, as is appropriate to meet the physical needs of the child.
• Create an effective discharge plan in collaboration with the child, family, DCFS Case Worker, and TFC Program Director.
• Assist Program Director with the provision of pre-service training, orientation, and in­ service training and consultation services for TFC parents.
• Coordinate targeted recruitment of foster parents, with a specific focus on identifying families that can meet the specialized needs of the youth in care.
