JOB DESCRIPTION:
Ryan White Medical Case Manager to provide case management services to individuals living with HIV/AIDS. Major responsibilities include coordination of services, assessments, service plans, clinic and home visits, and consumer advocacy
JOB REQUIREMENTS:
Bachelor’s degree in Human Services field; OR one year of paid post graduate work experience in direct service to HIV target population Preferred. Excellent writing skills and knowledge of various community resources required. Bilingual, Spanish /English a plus.
