JOB DESCRIPTION:
Full time Housing Case Manager is needed to monitor service delivery for veterans. Major responsibilities include coordination of services, assessments, and housing service plans. The Case Manager will be required to develop and maintain partnerships with local property owners and landlords and to maintain regular contact with caseload via phone calls, appointments & home visits. Local travel using personal vehicle is required and is estimated to make up approximately 50% of job duties. Mileage reimbursement will be provided.
JOB REQUIREMENTS:
Bachelor’s degree in a Human Service field and one year of post graduate work experience with veterans and/or individuals with disabilities is required. Candidates must have excellent communication skills and knowledge of various community resources. Reliable transportation, a valid driver’s license and current vehicle insurance is a must.
