The Community Support Specialist (CSS):
- Provides one-on-one services for adults and/or children with behavioral health disorders; providing training and support that promote recovery. Services are primarily provided in the community
- Conducts assessments of persons served, completes intakes and develops individual service plans
- Monitors the following community living areas: housing, entitlements, education, finances, social/recreational, family living, mental and physical health, legal assistance
- Provides and documents job mandated services to meet current program budgetary goals
- Links persons served to needed services and acts as an advocate as needed
- Creates, develops, and maintains linkages with other service providers and community resources that may be of assistance to persons served
- Manages all case records in accordance with licensing and contractual requirements using an Electronic Health Record
- Develops and maintains positive relationships with funding and regulatory agencies
- Flexible schedule
Position Requirements:
- Bachelor's Degree in psychology, social work, counseling or sociology required
- Experience in the provision of direct client services in a social service related field preferred
- Must have safe, reliable transportation and ability to transport persons serviced in personal vehicle within program service area
