Description
Volunteers of America is looking to hire a Medical Case Manager for our HIV/AIDS program in the Baton Rouge area.
Medical Case Managers provide a range of client centered services to link HIV positive clients with health care, psychosocial, and other services. These services ensure timely and coordinated access to medically appropriate levels of health and support services and continuity of care, through ongoing assessment of the client’s and other key family members’ needs and personal support systems.
JOB REQUIREMENTS:
Provide treatment adherence counseling to ensure readiness for and adherence to complex HIV/AIDS treatments
Complete initial assessments of services needed
Develop comprehensive and individualized service plans
Coordinate services required to implement the service plans
Provide continuous client monitoring to assess the efficacy of the plan
Provide periodic reevaluation and adaptation of the service plan, every 6 months, as necessary during the enrollment of the client
Conduct home and clinic visits to ensure medical compliance
Maintain required documentation (daily service logs, annual reporting, etc.)
Follow all guidelines & procedures related to EZ Labor (clocking in & out, reviewing time card, requesting edit sheets, etc)
Report any significant concerns to supervisor
Maintain training requirements
Projects and duties as assigned
EDUCATION AND EXPERIENCE:
A bachelor’s (required) or master’s degree (preferred) in social work from a program accredited by the Louisiana State Board of Social Work Examiners; OR
• A bachelor’s (required) or master’s degree (preferred) in a human service related field OR
• One (1) year of paid post-degree experience in direct service to HIV target population. Good computer, communication, interpersonal, and writing skills are required as is ability to work independently. Job involves local travel is required so a valid driver’s license and vehicle insurance is required.
