Description
Volunteers of America is looking to hire a Non-Medical Case Manager for our HIV/AIDS program in the Baton Rouge area.

The Non-Medical Case Manager will provide guidance and assistance in accessing medical, social, community, legal, financial, and other needed services. Non-Medical Case management services may also include assisting eligible clients to obtain access to other public and private programs for which they may be eligible, such as Medicaid, Medicare Part D, State Pharmacy Assistance Programs, Pharmaceutical Manufacturer’s Patient Assistance Programs, other state or local health care and supportive services, or health insurance Marketplace plans. Methods of communication include face-to-face, phone contact, and any other forms of communication deemed appropriate. 

JOB REQUIREMENTS:
Provide direct services to program individuals utilizing Ryan White Part A funds
Assist program individuals in removing barriers to care
Assist with enrolling program individuals in LAHap.
Assist program individuals in applying for Medicaid/Medicare benefits
Complete reassessments on program individuals in accordance with regulatory guidelines
Maintain contact with program individuals in accordance with regulatory guidelines
Assist program individuals in obtaining community support, resources and services
Maintain required documentation (daily unit logs, annual reports etc) 
Follow all guidelines & procedures related to EZ Labor (clocking in & out, reviewing time card, requesting edit sheets, etc)
Report any significant concerns to supervisor
Maintain training requirements
Projects and duties as assigned

EDUCATION AND EXPERIENCE:
Bachelor’s degree in the Human Service Field. Valid driver’s license, excellent writing and computer skills required. A minimum of one year paid work experience with persons with HIV/AIDS or other catastrophic illness preferred; and/or
• State or National certification from a recognized state/national certification organization and/or licensing organization preferred (i.e. LBSW, LMSW, LCSW, LPC, LMFT, LCDC, etc.)
• Knowledge and training in assessment of needs, formulation of care plans, monitoring of care plans and evaluation of case pro files; and
• Extensive knowledge of community resources and services
• Job involves local travel. A valid driver’s license and vehicle insurance is required.
