Description
We are offering a $1,000 Sign on Bonus
JOB DESCRIPTION:
Provide CART screening interview to persons that call or present to the clinic in crisis.
Collect necessary data to assess crisis needs, including demographics, imminent danger, details of child/youth
crisis event, substance abuse issues. Input this information into electronic health record
Make a recommendation for disposition to next, least restrictive, appropriate level of care or needed referral source
Maintain a person centered approach, understanding that the child/youth crisis is determined by the caller
Complete all needed paperwork concurrently to assist with the resolution of the crisis event
Promptly record all interactions on Service Log
Verbal staffing with face to face assessor when a face to face visit is needed
Bachelor's Degree in a Social Service field or experience preferred.
