Description
Volunteers of America South Central Louisiana is seeking to hire a Housing Case Manager in our HOPWA program in the Baton Rouge area.

The Housing Case Manager will provide housing case management to clients by assisting clients in accessing needed services.

ESSENTIAL FUNCTIONS:
  1. Assist Housing Coordinator with interviewing clients applying for Tenant Based Housing Program
  2. Complete intake process for housing programs once client is accepted
  3. Complete housing care plans, assessments/reassessments (every 6 months)
  4. Complete client referrals to community resources
  5. Assist clients with employment applications, GED/educational enrollments, money management, etc.
  6. Maintain monthly contact with Tenant Based and Project Based Housing participants in accordance with regulatory guidelines
  7. Complete a discharge plan with Tenant Based Housing participants
  8. Works closely with housing participants' medical case manager and/or service coordinators
  9. Maintain client files
  10. Assist Program Director and Housing Coordinator with the completion of monthly, quarterly, and annual reports
  11. Follow all guidelines & procedures related to EzLabor (clocking in/out, reviewing time card, requesting edit sheets, etc.)
  12. Report any significant concerns to Housing Coordinator and Supervisor
  13. Assist Housing Coordinator with preparation of quarterly tenant meetings and annual property activities
  14. Attend staff and team meetings as well as maintain training requirements
  15. Projects and other duties as assigned

EDUCATION AND EXPERIENCE:
  1. Bachelor's degree in Human Service field
  2. Good computer communication, interpersonal, and writing skills required  
  3. Ability to work independently
