Description
Volunteers of America South Central Louisiana is seeking to hire a Case Manager in our Homeless Initiative Program in the Baton Rouge area.

The HIP case manager is responsible for monitoring service delivery of the caseload of clients who are  enrolled in our Homeless Initiative Program (HIP). 

JOB DUTIES:
* Review referrals and assess program eligibility 
* Complete intake and initial assessment of services needed 
* Assist with developing and maintaining monthly budgets (where applicable) for enrolled program individuals
* Develop, monitor, and maintain service plans in accordance with program guidelines
* Assure that units are leased within budget guidelines
* Maintain contact with program individuals accordance to program guidelines 
* Assist program individuals in obtaining community support, resources, and services


JOB REQUIREMENTS:
A bachelor's Degree is required, and at least  1-3 years of experience working with the homeless population or individuals with a disability. The individual must be able to maintain professional, courteous relationships with all consumers, staff, community members and professional partnerships.
