JOB SUMMARY:
 
Manage housing stabilization services and the individual service plan for Permanent Supportive Housing (PSH) tenants.
 
JOB DUTIES:
General responsibilities:
  • Manage housing stabilization services, and the individual service plan
  • Conduct strength based needs assessment
  • Develop, implement, monitor, evaluate and modify a person-centered service plan
  • Problem solve conflicts
  • Coordinate PSH housing support services with other services, and with other members of Housing Support Team (HST)
  • Maintain assigned program record
  • Provide and document job mandated services and billable (face-to-face with person served) units to meet current program budgetary goals
  • Participate in on-call rotation and provides 24-hour emergency access to persons served according to procedures
  • Attend all required trainings and meetings and all other duties as assigned
 
Pre-Tenancy Assistance:
  • Conduct an initial assessment of housing preferences and housing history
  • Assist individuals in determining housing preferences if needed
  • Assist individuals in compiling housing information needed for housing applications (if needed) such as credit reports, landlord references, income and asset documentation
  • Assist individuals in obtaining, completing and submitting housing applications
  • Assist individuals in attending any meetings with housing provider including providing or arranging transportation and attending meetings as needed
  • Assist individuals in filing appeals and requests for reasonable accommodation under Fair Housing Law
 
Move-In:
  • Assist tenants to understand basics of landlord/tenant law and lease requirements
  • Assist tenants to access Housing Establishment and Preservation Fund for security deposit, securing furniture and other households items if needed
  • Assist tenants with moving-in activities
  • Assist tenants to establish utilities and telephone
  • Assist tenants in orienting to home and to neighborhood
  • Identify any housing or community living related skills training needed including home maintenance, shopping, cooking, and budgeting
  • Provide community living skills training or make appropriate referrals
 
Tenancy Stabilization:
  • Establish routine contact with tenants to ensure early detection of any housing issues
  • Provide ongoing housing-related skills training if needed and identify any issues that may require additional training by other parties
 
Service Coordination:
  • Establish contact and working relationship with tenant’s other service provider(s) and participate on the tenant’s treatment or services team when needed
  • Assure tenant is referred and/or make referrals for benefits including but not limited to SSI, Food Stamps and Medicaid, medical treatment, substance abuse treatment, mental health services, personal care attendant services, employment or education related activities, support groups or other services based on protocols with primary service provider
  • Assume responsibility for or establish relationship with agency that has 24/7 crisis support responsibility including developing a crisis plan and intervening with each tenant as set forth in the crisis plan and written protocol with primary service provider
 
JOB SPECIFICATIONS:
 
  • Bachelor's degree in a social service related field
  • Experience in the provision of direct client services or case management in a social service related field preferred
  • Must have safe, reliable transportation and ability to transport individuals in person vehicle within program service area
 
PHYSICAL REQUIREMENTS:
While performing the duties of this job, the employee is frequently required to stand; walk; sit, use hands, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. While performing the duties of this job, the employee can sometimes be exposed to weather conditions. The noise level in the work environment is usually moderate.
