JOB DESCRIPTION:
Responsible for providing recovery based, client centered support services using the most effective available resources in meeting the identified service plan goals for each consumer. The individual acts as liaison between treatment providers and the client. The case manager assists clients with the development of social and community support systems that promote stability and enable the client to function at the highest level possible. Case Managers offer their extensive knowledge of community resources and support as a primary means of assisting each client to reach their rehabilitation and recovery goals.

JOB REQUIREMENTS:
• Provide case management and referral services for an assigned group of consumers including coordinating and monitoring the activities of the individual treatment team; assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in collaboration with the consumer and with OBH treatment team; ensure immediate changes are made in treatment plans a client’s needs change; educate and support clients’ families; and advocate for clients’ rights and preferences. 
• Provide brokerage, linkage and advocacy to ensure resources and services are accessed in a timely manner, providing community based services in client’s residence as needed.
• Assist clients in adhering to treatment plans, such as setting up appointments, arranging for transportation to appointments, or providing support. 
• Conduct short-term individual counseling including crisis intervention
• Develop, prepare, and maintain required charting and documentation in a timely and thorough manner. 
• Develop Individualized Service Plans with measurable goals and objectives and assist with developing WRAP Plans.  
• Develop relationships with representatives in other agencies to support individuals in attaining services such as housing, additional mental health care, psychiatric medication, medical resources, financial assistance, legal advocacy, etc. 
• Conduct comprehensive assessments for mental health and/or substance abuse treatment. 
• Provides on-call crisis intervention covering nighttime hours by team rotation .
• Provide individual services to consumers on a weekly basis.  This includes regular face to face contact their home or community setting to prevent decompensation. 
• Document client progress to maintain a permanent record of client activity according to established methods and procedures. 
• Participate in staff organizational meetings and treatment planning review meetings. 
• Assist consumers with monitoring medication adherence and educate on medication side effects, dosing and organizing pill boxes. 
• Attend training sessions on a regular basis.  Case Manager is responsible for obtaining forty hours of training per fiscal year. 
• Adhere to all client confidentiality requirements and standards. 
• Complete monthly progress reports for each consumer.
• Maintain and submit proof of active auto liability insurance for privately-owned vehicles used to conduct agency business and/or transport agency clients.  Employees must submit proof of insurance prior to employment and each time policy is renewed thereafter.  Failure to maintain active coverage and submit proof may result in disciplinary action. 
• Transport clients as directed and follows agency standards, rules, regulations, and all laws for safe transport.
