Job Description:
The QIDP will develop & monitor person-centered plans of care for individuals with disabilities. Specific duties include: writing Individual Program Plans & conducting meetings, developing & monitoring skills training programs, and gathering, reviewing & monitoring reports from physicians, therapists & employers Other responsibilities include: providing 24 hours on-call supports for client behaviors, hospitalizations etc.
Requirements:
Bachelors degree in social service field, good organizational skills & Minimum 3 years experience in the above specified areas
