JOB DESCRIPTION:
The Ryan White Medical Case Manager will provide case management services to individuals living with HIV/AIDS. Major responsibilities include coordination of services, assessments, service plans, clinic and home visits, and consumer advocacy.
JOB REQUIREMENTS:
Bachelor’s degree in social services or human services field, OR one year of paid post graduate work experience in direct service to HIV target population. Excellent writing skills and knowledge of various community resources required. Bilingual, Spanish /English a plus.
